All Filipinos are required to report important birth of their children, change of civil status such as marriage, which the Embassy/Consulate forwards to the Philippine Statistical Authority (formerly NSO).

Please note that our office is only able to Consularise your application forms - they will then need to be sent to the Embassy to be processed. ​

IMPORTANT: The Consulate receives payment in cash in AUD only.

Please bring the following:

1. TWO (2) Forms of VALID GOVERNMENT ID's at least (1) Primary and (1) Secondary ID or (4) Secondary ID's such as:

2. Please download the Report of Birth form below.

  • IMPORTANT: Do not sign the Report until you attend our office for witnessing.
  • Please provide 4 copies to our office for signing.
  • The form must be accomplished neatly. No erasures please as this will be the permanent record. The Embassy has the discretion to reject forms that are sloppily accomplished or with erasures or with wrong entries and with wrong stamp by the notary public.
  • ​Please ensure to bring $45AUD in cash for our office to notarize your report of birth
  • Please bring self-addressed express post envelope for the return of your notarized document(s).

3. IT IS THE APPLICANT'S RESPONSIBILITY TO SEND the Report of Birth Application to the Philippine Embassy in Canberra along with the following for processing:

  • Original Birth Certificate of the child with registry number issued by the Civil Registrar or Registry of Births, Deaths and Marriages of the state/territory where the birth was registered plus four (4) photocopies duly certified by the Justice of the Peace (JP).
    • Note: Original will be returned together with the official copy of the Report of Birth of your child.
  • Original PSA issued Marriage Certificate (or original Australian Marriage Certificate with registry number) of parents plus four (4) photocopies duly certified by the JP.
    • Note: Original will be returned together with the official copy of the Report of Birth of your child.
  • Four (4) photocopies of the data page of valid passports of parents or other form of identification, i.e. driver’s license if valid passport copy is not available duly certified by the JP..
  • ​Please ensure to pay $45.00 either in money order or bank cheque to the Philippine Embassy

Additional Requirements for Report of Birth

For late registration:

1. If reporting 1 year after the child’s birth, an Affidavit of Late

2. If reporting 5 years after the date of birth, a recently-issued original PSA certificate of no report of birth record is required.

If legitimated (if parents were married after the child’s birth):

1. Joint Affidavit of Legitimation executed and signed by both parents and duly notarised by Australian notary public/lawyer.

2. Submit one (1) original and three (3) photocopies.

If Illegitimate and Using Father’s Surname:

IMPORTANT NOTE: If the father OR mother is not in Australia, this document must be notarised and registered either in the Philippine embassy or consulate in the country where he resides or if he is in the Philippines by a notary public and it will be registered by the local civil registrar where the birth of the child is to be registered.

The Embassy will issue the Certificate of Registration upon submission of the Affidavit of Admission of Paternity (AAP) and the Affidavit to Use Surname of the Father (AUSF).

1. Four (4) original Affidavit of Admission of Paternity (AAP) duly executed by the child’s father, to be signed before a consular officer at the Embassy, if filed in person.

2. Copies of father’s valid passport and the unregistered birth certificate of the child are required.

3. Four (4) original Affidavit to Use Surname of the Father (AUSF), executed by the child’s mother, to be signed before a consular officer at the Embassy, if filed in person.

4. Copies of mother’s valid passport and the unregistered birth certificate of the child are required.

5. Certificate of Registration

If the Filipino parent is already a citizen of Australia after the birth of the child:

1. If the Filipino parent is already a citizen of Australia after the birth of the child

2. Four (4) certified true copies of the old Philippine passport

If the Filipino parent reacquired/retained his/her Philippine citizenship after the birth of the child:

IMPORTANT NOTE: In addition to the above-mentioned requirements, other documents might be required when deemed necessary by the Consular Officer of the Embassy.

1. Four (4) certified true copies of the documents pertaining to the re-acquisition/retention of his/her Philippine citizenship.

2. You are then to send the signed Report of Marriage to the Philippine Embassy in Canberra for processing along with the following:

  • Original Marriage Certificate with registry number issued by the Civil Registrar or Registry of Births, Deaths and Marriages of the state/ territory where the marriage took place plus four (4) photocopies duly certified by the Justice of Peace (JP).
    • Note: Original will be returned together with the official copy of the Report of Birth of your child.
  • For Filipino spouse/s: Original birth certificate issued by the Philippine Statistics Authority (PSA) plus four (4) photocopies duly certified by the Justice of Peace (JP).
    • Note: Original will be returned together with the official copy of your Report of Marriage.
  • Four (4) photocopies of the data page of valid passports of both parties or other form of identification, i.e., driver's license if valid passport copy is not available duly certified by the JP.
  • ​Please ensure to pay $45.00 either in money order or bank cheque to the Philippine Embassy

Additional Requirements for Report of Marriage (if Applicable):

For late registration:

1. If reporting 1 year after the marriage date, an Affidavit for the Delayed Registration of Marriage is required. The Affidavit should be properly filled up and signed at our Office to be then sent to the Embassy with a payment mentioned below.

2. If reporting 5 years after the date of marriage, a recently-issued original PSA certificate of no report of marriage record is required.

For the Filipino spouse with previous marriage/s in the Philippines:

1. For Filipino spouse with annulled marriage in the Philippines: Original Marriage Contract issued by the Philippine Statistics Authority (PSA) with proper annotation plus four (4) photocopies duly certified by the Justice of Peace (JP).

  • Note: Original will be returned together with the official copy of your Report of Marriage.

2. For Filipino spouse whose divorce abroad has been recognized in PH, original PSA Marriage Contract with proper annotation and/or Philippine Court recognition of a foreign divorce decree plus four (4) photocopies duly certified by the Justice of Peace (JP).

  • Note: Original will be returned together with the official copy of your Report of Marriage.

3. For widowed Filipino spouse: Original PSA Death Certificate of the deceased spouse, original PSA issued marriage certificate, plus four (4) photocopies of both documents duly certified by the Justice of Peace (JP).

  • Note: Original will be returned together with the official copy of your Report of Marriage.

For the foreign spouse with previous marriage/s:

1. For divorced foreign spouse: Four (4) photocopies of divorce order/certificate of divorce, duly certified by the Justice of Peace (JP) (if former spouse of the foreigner is a Filipino, original apostillized Judicial Decree of Annulment with Certificate of Finality, plus 3 photocopies.

  • Note: Original will be returned together with the official copy of your Report of Marriage.

2. For widowed foreign spouse: 4 photocopies of Death Certificate of previous spouse (if deceased spouse is a Filipino, original PSA Death Certificate plus four (4) photocopies duly certified by the Justice of Peace (JP).

3. Please note that in addition to the above-mentioned requirements, other documents might be required when deemed necessary by the Consular Officer of the Embassy.

Filing by mail can be done by doing the following:

1. Send all the requirements (including the notarised/witness Report of Birth/Marriage done at our office) and applicable additional documents along with the following:

  • AUD 45 payment for the Report of Marriage payable by a postal money order or bank cheque to the Philippine Embassy. If paying by credit card please provide the credit card name, number, expiry date and cvv number at the back of the card).

2. AUD 45 payment for Affidavit of Delayed Registration of Marriage, if applicable.

3. Pre-paid express return envelope (large size) with return address.

4. Please provide your cell phone number & email address to allow the Embassy to communicate with you regarding your application.

  • Please provide 4 copies to our office for signing.
  • The form must be accomplished neatly. No erasures please as this will be the permanent record. The Embassy has the discretion to reject forms that are sloppily accomplished or with erasures or with wrong entries and with wrong stamp by the notary public.
  • ​Please ensure to bring $45AUD in cash for our office to notarize your report of birth
  • Please bring self-addressed express post envelope for the return of your notarized document(s).
  • Please ensure to record your tracking number as the Consulate Office assumes no responsibility for any loss of your mail.

2. You are then to send the signed Report of Death to the Philippine Embassy in Canberra for processing along with the following:

  • Original Death Certificate issued by the State Registry showing the personal circumstances of the deceased and to be apostilled by the Australian Department of Foreign Affairs and Trade (DFAT) plus four (4) photocopies duly certified by the Justice of Peace (JP).
    • Note: Original will be returned together with the official copy of your Report of Death.
  • Original PSA issued Marriage Certificate (or original Australian Marriage Certificate with registry number) of parents plus four (4) photocopies duly certified by the JP.
    • Note: Original will be returned together with the official copy of the Report of Birth of your child.
  • Four (4) photocopies of the data page of valid passports of parents or other form of identification, i.e. driver’s license if valid passport copy is not available duly certified by the JP..
  • ​Please ensure to pay $45.00 either in money order or bank cheque to the Philippine Embassy.

Consular Mortuary Certificate Requirement

For Human remains that has not been cremated will be shipped to the Philippines:

1. Duly accomplished and signed Request form for Consular Mortuary Certificate applications.

2. Please be in our office for signing.

3. Original Death Certificate issued by the State Registry showing the personal circumstances of the deceased and to be apostilled by the Australian Department of Foreign Affairs and Trade (DFAT), plus three (3) certified true photocopies.

4. Original Mortuary/Funeral Director’s Certificate, stating that the body was properly embalmed/cremated in accordance with internationally-accepted health practices; that it is in a sanitary condition consistent with existing standards for overseas shipment; and that the casket/urn has been hermetically sealed and contains only the remains of the deceased, plus three (3) certified true photocopies.

5. Original Certificate of Non-Contagious Disease issued by the Local Area Health Service, stating that the deceased did not die of a contagious disease or that the remains to be shipped did not originate from a place contaminated by a contagious disease, plus three (3) certified true photocopies.

6. Original Transit Permit issued by the Local Area Health Service at the place where the transportation begins, plus three (3) certified true photocopies.

7. Proof of Identity of the Deceased (if non-Filipino): Four (4) certified true copies of the Australian passport.

8. Proof of Filipino citizenship of the deceased, if applicable: Four (4) certified true copies of the deceased’s Philippine passport; Australian Permanent Resident Card or Work Permit, or Dual Citizenship documents (Identification Certificate and Approval Order).

9. Information on the consignee/addressee/person, name of carrier, date and port of embarkation, port of destination and scheduled time of arrival.

10. Please ensure to pay $45.00 CASH to the Philippine Consulate Queensland

11. Affidavit of Delayed Registration of Death is required if application is filed more than one (1) year after the occurrence.

For COVID-19 related deaths:

1. The remains shall be cremated before shipment.

2. For remains that do not require cremation, the casket shall be permanently and hermetically sealed from the port of origin until burial. The burial must take place within twenty-four (24) hours after the issuance of clearance of admission into the country

3. Original Death Certificate issued by the State Registry showing the personal circumstances of the deceased and to be apostilled by the Australian Department of Foreign Affairs and Trade (DFAT), plus three (3) certified true photocopies.

4. Proof of Identity of the Deceased (if non-Filipino): Four (4) certified true copies of the Australian passport.
